Stay with Dr. Josh for your ongoing care. Concierge Membership lets in-network insurance patients keep their follow-up visits with Dr. Josh instead of moving to an associate provider, with priority scheduling to fit your schedule.
Membership includes
- Ongoing follow-up visits with Dr. Josh
- Priority scheduling
- More frequent reassessment and plan progression than the standard insurance-based schedule
- After-hours communication by email or phone, as determined by your physical therapist
Eligibility
Available to in-network insurance patients. Not available to Medicare or Medicare Advantage patients.
Billing
Billed monthly, directly to you, on the same day each month. The membership is never billed to insurance. Evaluations and progress reassessments are still billed to your insurance.
*The membership fee is in addition to any co-pay or co-insurance under your health plan and does not affect your insurance coverage or benefits.

Single visit – $275
10-visit package – $2,150 ($215 per visit)
Home visits are available to concierge patients and are priced by distance from the clinic. Please call the office for home visit pricing.

Who is eligible for Concierge Membership?
In-network insurance patients who want to continue their ongoing follow-up visits with Dr. Josh. Medicare and Medicare Advantage patients are not eligible.
Is the membership fee billed to my insurance?
No. The $500 monthly fee is billed directly to you on the same day each month and is never billed to insurance. Evaluations and progress reassessments are still billed to your insurance, and the fee is in addition to any co-pay or co-insurance.
What does the membership include?

Is there a concierge option if I'm paying cash?
Yes. Concierge visits are $275 each, or $2,150 for a 10-visit package ($215 per visit).
Do you offer home visits?
Yes, for concierge patients. Home visits are priced by distance from the clinic, so please call the office for pricing.
Does membership change my insurance coverage?
No. Your insurance coverage and benefits stay the same and continue to apply to your covered visits.
